Certificate of Compulsory School Attendance (for State Driving Test)

If you are applying for a Tennessee driver's permit or driver's license, you will need a signed Certificate of Compulsory School Attendance from Liberty Creek High School. Simply complete this SF-1010 form and bring it to Jodi Curtis, the attendance clerk, or send your email request to Jodi Curtis. You may pick your signed letter up in the front office at the end of the school day. This letter must be dated no more than 30 days prior to your Tennessee driver's application.

Simply fill out the Application Form and bring your $5 to the front office to get your student parking pass or pay using Online School Payments.
